A newly released report from the Department of Health and Human Services alleges a coordinated pattern of insurance fraud within the pediatric gender medicine industry, with providers accused of misusing diagnostic codes to bill insurers for treatments including hormones, puberty blockers, and surgeries on minors. The findings, released Aug. 13, have already prompted Vice President JD Vance to call for a Department of Justice investigation.

While the allegations remain unproven and organizations named in the report have pushed back, the findings raise serious questions about how gender procedures for children have been marketed, coded, and financed across the American healthcare system.

The report, titled “Wolves in White Coats: How Doctors and Hospitals Pushed and Profited from the Fraud of ‘Gender Medicine,'” examined the actions and motives of 225 healthcare institutions and found they allegedly put profits and gender ideology ahead of patient needs.

“How do you do a mastectomy, but then bill an insurance company and not raise any red flags? … You bill it as breast reduction instead of a mastectomy,” said Dr. Eithan Haim, an author of the report.

“These allegations are crazy, and they demand investigation,” Vance said in a video posted to social media on Aug. 13.

According to the report, providers allegedly used vague or inaccurate insurance codes, such as “unspecified endocrine disorder,” to bill for cross-sex hormones, generating more than $42 million between 2015 and 2025 despite a University of Iowa study finding only a small fraction of those diagnosed actually had the condition. Similar patterns were found with puberty blocker billing under a “precocious puberty” diagnosis for teenagers, despite that age range being the natural onset of puberty. The report alleges that advocacy organizations, including WPATH and Planned Parenthood, encouraged providers to use these alternative codes, and that one toolkit produced by an advocacy group instructed providers on how to appeal insurance denials repeatedly until claims were approved. Neither WPATH nor Planned Parenthood responded to requests for comment from the outlet that first reported the story.

The report also found that the financial incentives behind pediatric gender procedures were substantial, citing patient costs ranging from $25,000 to over $170,000 depending on treatment, with the industry generating an estimated $120 million since 2019. The report further alleges that the Biden administration actively facilitated these procedures by threatening legal action against hospitals that declined to offer them, framing refusal as discrimination, and mandating coverage through federal insurance programs. President Donald Trump reversed these Biden-era policies at the start of his second term. Separately, the Federal Trade Commission has filed suit against WPATH, alleging the organization promoted youth gender interventions under false pretenses. WPATH has publicly disputed the FTC lawsuit, characterizing it as part of a broader campaign against gender-affirming care rather than a legitimate legal concern.

This report touches on deeply important questions of medical ethics, child welfare, and the protection of vulnerable young people from irreversible procedures. Scripture affirms the inherent dignity of every person as created in God’s image, male and female, and calls believers to protect children, who are often unable to fully grasp the lifelong consequences of medical decisions made in their youth. If these allegations of fraud prove true, they represent not only a financial scandal but a profound breach of trust toward families who believed they were receiving sound medical guidance.
